HIPAA Notice of Privacy Practices (NPP)

6. File a complaint – If you feel your privacy rights are violated, you may contact us or file a complaint with:

   • U.S. Department of Health and Human Services

   • Texas Behavioral Health Executive Council (BHEC)

3. Our Responsibilities

• We are required by law to maintain the privacy of your PHI.

• We must provide you this Notice of Privacy Practices.

• We will follow the terms of this Notice.

• If we change our policies, we will provide you with a revised notice.

4. Telehealth and Online Services

• All telehealth sessions are conducted through HIPAA-compliant platforms.

• We take technical and administrative safeguards to protect your information during online sessions.

5. Record Retention and Breach Notification (Texas Requirements)

• Retention: PHI is retained for a minimum of 7 years for adults, per Texas law.

• Breach Notification: If your PHI is accessed without authorization, you will be notified promptly, as required by law.
